El primer album de uno de los pioneros del death metal, que con sus guitarras arrolladoras, veloces baterias e increible voz se convirtió (en mi parecer y el de muchos) el mejor album de Morbid Angel. Aunque oficialmente este es el primer albúm, morbid angel ya tenia un Lp llamado "Abominations of Desolation" este no salio a la venta (luego salio en el año 1991) y fue entonces Altar of Madness el encargado de darle el exito a esta espectacular banda.
